number of landlord or mortgage servicer (and your loan number, for
the last 2 years)

* Current Employer
Name: Their address, phone number, your position/title

your current position/type of work.
* Last 30 days: Most
recent pay stubs
* Last 2 years: Last 2
tax year's W-2s
* If Self-Employed:
Most recent 2 year tax returns (with 1099s, K-1s or other relevant
attachments, etc.)
